Application for Leave of Absence:  Please use this form to apply for permission to take your child out of school during term time. All Leave of Absence applications MUST have an accompanying letter to explain the reason for the request.

Free School Meals & Pupil Premium - Eligibility and Application Form: Your completion of this form enables the school to check for Free School Meal Eligibility and also if additional money (Pupil Premium) can be claimed from the Government. Further information about how the school spends these monies can be found on the Pupil Premium page of our website.

Special Dietary Needs:  Please fill in the Special Dietary Request Form and return it to the school office if your child has any special dietary needs.
